The MD BoS HMIS Team is working to create PDF versions of HMIS assessments (Entry - Interim - Exit) by the different Project Types:

• HUD 
  • used by majority of projects - unless the project receives specific funding listed below
• PATH
• VA (including GPD and SSVF)
